If your child was Baptised in another parish please provide a copy of your child's Baptismal certificate to the Parish office.
Please email a scan or photo secretary@qop.nz

If your child was Baptised at Sacred Heart Addington, St Peters Beckenham, Our Lady of the Assumption Hoon Hay or Sts Peter & Paul Halswell you do not need to provide a Baptismal certificate. We can check our Baptismal Registers.

There will be a cost of $35 for this programme.

By ticking the box, I / We agree to our daughter's / son's enrolment for preparation for the Sacrament of Confirmation and to attending the information session and all other sessions with my/our child.

Missing sessions will mean that confirmation will be postponed to the following year.
