The Information Night is Thursday 19 Feb at Sacred Heart Hall 7:00pm (Parents only)

Our Programme will commence on the 25th or 26th of February.
All sessions will be at Sacred Heart Church and Hall.
Parents and children must attend all scheduled sessions.

Details of Baptism

If your child was Baptised in another parish please provide a copy of your child's Baptismal certificate to the Parish office.
Please email a scan or photo to secretary@qop.nz 

If your child was Baptised at Sacred Heart Addington, St Peters Beckenham, Our Lady of the Assumption Hoon Hay or Sts Peter & Paul Halswell you do not need to provide a Baptismal certificate. We can check our Baptismal Registers.

Cost is $50

By ticking the box, I / We agree to:

1. Our daughter's / son's enrolment for preparation for the Sacrament.

2. Attend all timetabled parent and or children meetings. I understand that attendance is mandatory for both the parent and the
    candidate. The candidate may not be able to receive the Sacrament if attendance and preparation is not done.

3. Support the programme at home, and help children with homework.

4. Pay for associated costs of my son/daughter participating in the programme.

5. Send a copy of my childs baptism Certificate to the Parish Secretary.

Parents permission to display photos

  1. I give permission for a picture of my child to be used on the Parish/diocesan publications.
  2. I give permission for my child to be in a group photo to be taken for the Parish website and/or Facebook page.
     

Privacy

  1. By submitting this form, I consent to my information being stored and used by Our Lady Queen of Peace Parish for the purposes of administering the Parish’s Reconciliation and First Holy Communion Programme and the pastoral needs of the families involved.
  2. I consent to my information being entered on the Parish Roll and I understand that all data will be held securely.
  3. I understand I have a right to access my information and amend, if required, and that when this information is no longer required for these purposes, my data will be disposed of according to the Parish Records Disposal Procedure.
